2005 Mt. Brewer Open - Homebrew Competition

                                        Saturday July 30^th at St. Cloud Park in Huntington, WV.  

Register online by filling out the form provided below.  Entries need to arrive between July 10^th and July 24^th. 

Entry fee is $5.00 per entry (2 bottles for each entry). 

Please complete the following steps to submit your entry.

• Fill out test boxes provided in the registration form.
• You can put up to five entries on one form.
• Click the Submit button when finished. This e-mails your form to Dave Zalewski.
• After submitting the entries, a summary page will sent to you via email.  Print this page and send it with your entries to:

• Plan shipping so entries arrive between July 10-24, 2005 
• Entry fees may be paid by check (Payable to the GHHA) or by credit card though PAYPAL. If paying by check, please include your check with your entries.
• Print registration form and attach labels to bottles Bottle Form
• Judges can register at Judge Registration Form
• Unless you are judging the event, entries must be arrive before 12:00 am Sunday July 23^rd, 2005.  Out of town judges may bring there beers to the competition site by 11:00 am July 30^th ,but all entries must be pre-registered.
